These terms and conditions, in accordance with the privacy policy, apply to SMP GO CARD. By applying for and/or using the SMP GO CARD, you agree to be following the terms and conditions outlined below. SERVICE AGREEMENT

• The SMP GO CARD and points, in whatever form they take, are issued by and remain the property of Multicare Health SDN BHD, which reserves the right to terminate, change, or amend the Plan's terms and conditions at any time.
• Individuals over the age of 18 are eligible for membership. Residents and non-residents of Malaysia (who must have a Malaysian telco number) may apply for membership.
• All Rewards and Gift Vouchers are subject to availability, as well as all applicable legal rules and Multicare Health Sdn Bhd's terms and conditions (including priority level, selection criteria, and liability limitations
• Points cannot be redeemed until they have been deposited to the Member's Membership account. Points will be entered in the Member's account only when Sunway Multicare Pharmacy shares the details of the relevant transaction for which Points are granted.
• Points expire 12 months after issuance on a first-in, first-out basis. Points are not transferable
• Multicare Health Sdn Bhd and the Merchants are not liable to Members for any indirect, consequential, or economic losses, loss of profits, loss of opportunity, or punitive damages of any kind.

The SMP GO CARD is not transferable and must be used by the cardholder-member only.

Users can no longer join or merge their accounts. Existing associate cardholders can keep their cards as associate cards. In the case that a member's associate card is misplaced or damaged. The card can be replaced by submitting an application for a new card.

Points

• Every RM1.00 transaction on SMP GO CARD earns one (1) Sunway point.
• SMP GO CARD is no applicable for bulk purchases.
• Multicare Health Sdn Bhd and Sunway Super App have the right to deduct points for any transaction or purchase that is returned in full or in part. This also applies to exchanges or products, unless the trade is for points-equivalent products.
• Individual promos may have various points as stated in their purchasing terms.
• The Points will be valid for one year from the date of the initial purchase. Any redemption of Points must be completed within one year.

Privacy Policy

Sunway Multicare Pharmacy / Multicare Health SDN BHD maintains your personal information private, including your name, email address, and phone number. Your personal information will not be disclosed or sold to third parties unless the following conditions are met:

• User has chosen to share with a third party; User has deliberately and willingly made public their personal information to enjoy products or services; User has breached the law
• Sunway Multicare Pharmacy's Terms and Conditions have been violated by the user.
• Sunway Multicare Pharmacy may use your contact information to send you emails about products and services that you might be interested in.
